Definition Recurring itch in the ear canal — sometimes with flaking or moisture — is often multifactorial: no clear infection or classic allergy, but an interplay of mast cell activation, histamine and a disturbed skin and mucosal microbiome. The gut-skin axis thereby connects gut flora, immune system and the sensitive skin of the ear canal.

In plain language

Recurring itch in the ears, sometimes with flaking or a little moisture, without any clear infection or allergy being found: ointments help for a while, but the itch returns. That's frustrating, because it seems to come out of nowhere. Yet there is a logic to it. The skin of the ear canal, like the rest of your skin, is connected to your immune system and your microbiome. Mast cells can release histamine locally there and thereby cause itch, even without a classic allergy. At the same time, the balance between ‘good’ and ‘troublesome’ bacteria in the ear canal may be disturbed, and histamine-rich foods or alcohol can intensify the complaints. The science behind

Chronic ear itch (pruritus auris) often proves multifactorial: overviews describe an interplay of immune and skin mechanisms rather than a single cause. Mast cells play a key role in this. They sit in the sensitive skin of the ear canal and release histamine when irritated, which triggers itch locally. Research into chronic itch places mast cells and histamine at the centre of its persistent character — even when no classic allergy or infection can be demonstrated.

The second layer is the gut-skin axis. The composition of the microbiome — in the gut and on the skin and mucous membranes — influences how strongly the skin inflames and how much histamine is available. A disturbed balance (dysbiosis) can feed or sustain skin and mucosal complaints such as ear itch. Nutritional triggers play a part: histamine-rich products and alcohol can raise the histamine load and thereby intensify the itch. In this way a stubborn pattern arises that does not lie in the ear alone, but in the broader network of skin, immune system and microbiome.
