Allergy or mast cell activation?

You get itching, redness, a bloated feeling or headache after certain meals, and you think of a food allergy. But the allergy tests stay negative — while the complaints keep coming back, especially under stress and after histamine-rich food such as wine, aged cheese or tomatoes. How can something resemble an allergy so closely without being one? The answer lies with the mast cells: immune cells that can release histamine, even without the classic allergic route. Your gut flora plays a part, because some bacteria produce histamine themselves and thereby raise the load, while stress and sleep deprivation amplify the reactions. This creates a broad, variable picture of complaints that is often labelled a ‘vague allergy’. Distinguishing a true allergy from mast cell activation helps you get a grip on what your body is trying to tell you.
The science behind
Mast cells are immune cells that can release histamine and other signalling substances. In a classic allergy this happens via IgE antibodies, but mast cells can also be hyperactive without that route — the picture that is called mast cell activation syndrome (MCAS) and that is often confused with food allergy. Because the released histamine acts on skin, gut and blood vessels, a broad palette of complaints arises: itching, redness, abdominal complaints and headache, without an allergy test showing anything.
The microbiome colours this picture. Certain gut bacteria produce histamine themselves, while others help to break it down; the balance between them co-determines the total histamine load. Research describes how the microbiome influences histamine production and mast cell activity, and how the host–microbiota interaction steers immune regulation. Stress and sleep deprivation also raise the histamine peaks, so the complaints appear in waves and are hard to trace back to a single food.
Why one person reacts fiercely and another does not is co-metabolic: it depends on the interplay between genetic predisposition (including in histamine breakdown), microbiome, metabolites and lifestyle. Genes colour the sensitivity in the background, but the whole determines whether and when the complaints appear. This reading is supportive and observational — not a medical diagnosis.
